    The pattern of relapse and survival of elective irradiation of the upper neck for stage N0 nasopharyngeal carcinoma

    <p>Abstract</p> <p>Background</p> <p>To investigate patterns of failure and survival rates of elective irradiation of upper neck in N0 nasopharyngeal carcinoma patients.</p> <p>Methods</p> <p>From February 1996 to November 2002, 97 patients without cervical lymph node involvement were admitted for radiotherapy alone. Before treatment, each patient underwent enhanced CT of nasopharynx and neck. All patients received radiotherapy to the nasopharynx, skull base, and upper neck drainage areas (including levels II, III, and VA). The upper neck was irradiated to a total dose of 50-56 Gy/25-28 fractions/5-5.6 weeks. For the primary tumor, 22 patients used conventional fractionation for a total dose of 70 Gy/35 fractions/7 weeks, and 75 patients used an accelerated hyperfractionationated schedule for a total dose of 78 Gy/60 fractions/6 weeks.</p> <p>Results</p> <p>The median follow-up of these 97 patients was 7.75 years. 10 patients had recurrences in the nasopharynx, 8 had distant metastasis, and 5 had recurrences in the cervical lymph nodes. Among the cervical lymph node failures, the areas of recurrence were in the II drainage areas in 4 patients who had neck dissections afterwards, and in IA drainage areas in 1 patient who also had recurrence in the nasopharynx. The causes of death were recurrence in the nasopharynx for 8 patients, 1 of these also had recurrence in the neck, distant metastases in 8 patients, and non-neoplastic diseases in 3 patients.</p> <p>Conclusions</p> <p>The causes of failure of N0 patients with nasopharyngeal carcinoma after radiotherapy alone to the nasopharynx and upper neck were nasopharyngeal recurrence, distant metastasis, and cervical recurrence in order of frequency. Elective irradiation of upper neck (II, III, VA) is advised for stage N0 patients diagnosed by enhanced CT of neck. Cervical recurrence alone is rare, which did not greatly affect the long-term survival after salvage neck dissection.</p

    The reversible addition fragmentation chain transfer (RAFT) polymerization method was adopted here to prepare a series of thermo-sensitive copolymers, poly (N,N-diethyl- acrylamide-b-N-vinylpyrrolidone). Their structures, molecular weight distribution and temperature sensitivity performances were characterized by the nuclear magnetic resonance (1HNMR), the gel permeation chromatography (GPC) and the fluorescence spectrophotometer, respectively. It has been identified that the synthesis reaction of the block copolymer was living polymerization. The thermo-sensitivity study suggested that N-vinylpyrrolidone (NVP), played a key role on the lower critical solution temperature (LCST) performance

    Evidence of Holocene climatic change and human impact in northwestern Yunnan Province: High-resolution pollen and charcoal records from Chenghai Lake, southwestern China

    High-resolution pollen and charcoal records from Chenghai Lake, northwestern Yunnan Province of China, are presented in this study. These records reveal changes in climate and human activity since 8220 cal. yr BP, and the drivers of climatic change are further discussed based on spectral analysis of Tsuga and evergreen oak pollen percentages. The pollen record indicated that the climate in the low altitude area was relatively warm and moderately dry, whereas the climate in the high altitude area was relatively cool and humid during the period 8220–4670 cal. yr BP, corresponding to the Holocene climate optimum, although the seasonality of precipitation increased between 5180 and 4670 cal. yr BP. From 4670 to 3470 cal. yr BP, the temperature declined, and the precipitation and its seasonality decreased. Between 3470 and 1860 cal. yr BP, the temperature was still relatively low, and precipitation appeared to decrease further. At the same time, human activity represented by agricultural planting began to influence this study area, but the intensity of human activity was relatively weak. After 1860 cal. yr BP, the climate was gradually becoming drier and the intensity of human activity such as deforestation and slash-and-burn agriculture increased. Spectral analysis of Tsuga and evergreen oak pollen percentages shows statistically significant periodicities of ~1500 and ~200 years at the ≥95% confidence level, which suggests that insolation and feedback interactions on millennial to centennial scales are the primary forcing mechanisms of the southwest monsoon and past climatic change during the Holocene in northwestern Yunnan Province, southwestern China.This research was financially supported by the Program of Global Change and Mitigation (2016YFA0600501), the National Natural Science Foundation of China (41572149, 41272188, 41372184), and Nanjing Institute of Geography and Limnology, CAS (NIGLAS2011KXJ02)

    Changes of Ecosystem Service Value in a Coastal Zone of Zhejiang Province, China, during Rapid Urbanization

    Gains and losses in ecosystem service values (ESV) in coastal zones in Zhejiang Province during rapid urbanization were analyzed in terms of land-use changes. Decision-making on coastal development based on ESV estimation is significant for the sustainable utilization of coastal resource. In this study, coastal land-use changes in Zhejiang Province during rapid urbanization were discussed based on remote-sensing derived land-use maps created in the years 1990, 2000 and 2010. The ESV changes in coastal zones in Zhejiang Province from 1990 to 2010 were estimated by using the established ESV estimation model. The analysis results demonstrate the following: (1) with the continuous acceleration of urbanization, land-use types in coastal zones in Zhejiang Province changed significantly from 1990 to 2010, demonstrated by considerable growth of urban construction land and reduction of forest land and farmland; (2) in the study period, the total ESV in coastal zones in Zhejiang Province continuously decreased in value from RMB 35.278 billion to 29.964 billion, a reduction of 15.06%; (3) in terms of the spatial distribution of ESV, the ESVs in coastal zones in Zhejiang Province were generally converted from a higher ESV to a lower ESV; (4) estimates of ESV for the three years 1990, 2000 and 2010 appear to be relatively stable; and (5) land-use intensity in coastal zones in Zhejiang Province continuously increased during the 20 years. The spatial distribution of land-use intensity was consistent with that of the ESV change rate.     Rapid Screening of Proanthocyanidins from the Roots of Ephedra sinica Stapf and its Preventative Effects on Dextran-Sulfate-Sodium-Induced Ulcerative Colitis

    Proanthocyanidins (PACs) have been proven to exert antioxidant and anti-inflammatory effects. In this study, ultra-performance liquid chromatography (UPLC) coupled with linear ion trap-Orbitrap (LTQ-Orbitrap) high-resolution mass spectrometry was first employed to systematically screen PACs from the roots of Ephedra sinica Stapf, and its ethyl acetate extract (ERE) was found to contain PAC monomers and A-type dimeric proanthocyanidins, which were tentatively identified through characteristic fragmentation patterns. In vitro, the antioxidant activity of ERE was tested through 2,2-diphenyl-1-picrylhydrazyl (DPPH) and 2, 2-azino-bis-3-ethylbenzothiazoline-6-sulfonic acid (ABTS) assays. In addition, ERE could inhibit the production of nitric oxide (NO) in lipopolysaccharide (LPS)-induced RAW 264.7 cells. In vivo, the preventative effects on dextran-sulfate-sodium-induced ulcerative colitis in mice was investigated. Mice were administered with ERE for 21 days, and during the last 7 days of the treatment period dextran sulfate sodium (DSS) was used to induce experimental colitis. The results showed that ERE treatment alleviated DSS-induced colitis, which was characterized by decreases in disease activity index (DAI) scores, spleen index and colon levels of TNF-&alpha; and IL-6, mitigation in pathological damage and oxidative stress and increases in colon length and IL-10 levels. In conclusion, supplementation of PACs derived from ERE may offer a new strategy for the treatment of ulcerative colitis. Moreover, our research will greatly facilitate better utilization of Ephedra plants

    Suppression of lysosome function induces autophagy via a feedback down-regulation of MTOR complex 1 (MTORC1) activity

    Autophagy can be activated via MTORC1 down-regulation by amino acid deprivation and by certain chemicals such as rapamycin, torin, and niclosamide. Lysosome is the degrading machine for autophagy but has also been linked to MTORC1 activation through the Rag/RRAG GTPase pathway. This association raises the question of whether lysosome can be involved in the initiation of autophagy. Toward this end, we found that niclosamide, an MTORC1 inhibitor, was able to inhibit lysosome degradation and increase lysosomal permeability. Niclosamide was ineffective in inhibiting MTORC1 in cells expressing constitutively activated Rag proteins, suggesting that its inhibitory effects were targeted to the Rag-MTORC1 signaling system. This places niclosamide in the same category of bafilomycin A1 and concanamycin A, inhibitors of the vacuolar H(+)-ATPase, for its dependence on Rag GTPase in suppression of MTORC1. Surprisingly, classical lysosome inhibitors such as chloroquine, E64D, and pepstatin A were also able to inhibit MTORC1 in a Rag-dependent manner. These lysosome inhibitors were able to activate early autophagy events represented by ATG16L1 and ATG12 puncta formation. Our work established a link between the functional status of the lysosome in general to the Rag-MTORC1 signaling axis and autophagy activation. Thus, the lysosome is not only required for autophagic degradation but also affects autophagy activation. Lysosome inhibitors can have a dual effect in suppressing autophagy degradation and in initiating autophagy

    Extra-anatomic revascularization and a new cannulation strategy for preoperative cerebral malperfusion due to severe stenosis or occlusion of supra-aortic branch vessels in acute type A aortic dissection

    Objectives: Acute type A aortic dissection (ATAAD) with severe stenosis or occlusion of the true lumen of aortic arch branch vessels often leads to an increased incidence of severe postsurgical neurological complications and mortality rate. In this study, we aimed to introduce our institutional extra-anatomic revascularization and cannulation strategy with improved postoperative outcomes for better management of patients with cerebral malperfusion in the setting of ATAAD. Methods: Twenty-eight patients with ATAAD complicated by severe stenosis or occlusion of the aortic arch branch vessels, as noted on combined computed tomography angiography of the aorta and craniocervical artery, between January 2021 and June 2022 were included in this study. Basic patient characteristics, surgical procedures, hospitalization stays, and early follow-up results were analyzed. Results: The median follow-up duration was 16.5 months (interquartile range: 11.5–20.5), with a 100% completion rate. The 30-day mortality rates was 7.1% (2/28 patients); two patients had multiple cerebral infarctions on preoperative computed tomography and persistent coma. Postoperative transient neurological dysfunction occurred in 10.7% (3/28) of the patients, and no new permanent neurological dysfunction occurred. Of all the patients, 3.6% (1/28) had novel acute renal failure. No other deaths, secondary surgeries, or serious complications occurred during the early follow-up period. Conclusions: Use of extra-anatomic revascularization and a new cannulation strategy before cardiopulmonary bypass is safe and feasible and may reduce the high incidence of postoperative neurological complications in patients with ATAAD and cerebral malperfusion
